Just a gentle reminder, please wear or bring comfortable clothing and have your intake form completed prior to your scheduled appointment. If you have any additional information, such as requisitions, imaging reports or anything else you believe would be of relevance, please bring it with you.

Just a reminder we do not direct bill. All appointments are to be paid at the end of every appointment. You will receive a receipt to submit to your insurance. The average time for reimbursement is typically 2 business days.
