Your First Appointment at ETHOS Collective Physiotherapy
We can’t wait to meet you and support you on your health and wellness journey! Please read the information below before your first appointment to ensure everything runs smoothly. If you have any questions, don’t hesitate to contact us.
Initial Assessment – What to Expect
Your first session is a one-on-one appointment, lasting approximately 1 hour. Here’s what it includes:
- Medical History – We gather your personal and health history to understand your body and goals. 
- Full-Body Evaluation – A physical assessment of your posture, movement, and the condition you’re seeking help for. 
- Findings & Treatment Plan – We review what we discover and outline a treatment approach tailored to you. 
- Personalized Exercises & Recommendations – Targeted strategies to address deficiencies and support your recovery and overall wellness. 
Tip: Please arrive a few minutes early and have your initial assessment and consent form completed.
Billing & Payments
- Payment is collected at the end of every session. 
- We do not direct bill extended health benefits; receipts are provided for insurance reimbursement. 
- We do not accept MVA or WorkSafeBC claims. 
- Payment can be made via credit card, cash, or e-transfer, and a valid card on file is required. 
- Your personal and billing information is securely stored in an encrypted system. 
CHECKLIST BEFORE YOUR FIRST APPOINTMENT
Forms
I have completed the Initial Assessment & Consent Form online before my appointment, so my physiotherapist can review it ahead of time.
Be prepared
For home physiotherapy visits, I’ve cleared a quiet space and am ready to start on time. I’ve changed into comfortable physiotherapy clothing to maximize my session.
Be organized
I’ve written down questions for my physiotherapist and gathered any relevant documentation (e.g., physician notes, referrals, imaging) that may help with my assessment. I will be punctual. If I need to cancel, I’ll provide at least 48 hours notice to avoid a cancellation fee.
